Diabetes Screening

Fellow Lions we can undertake Diabetes Screening thanks to Lloyds Pharmacy, there is a process that needs to be followed to allow this to happen.  Below are guidance notes on how to complete such a screening and also the request form for assistance from Lloyds Pharmacy.
